To make a referral for Vocational Rehabilitation or Disability Management services, please provide a referral letter or email outlining the specific service requested, the reason for the service, and if available the following information:

Limitations/restrictions obtained from objective data such as:

• Functional Capacity Evaluation

• Independent Medical Evaluation

• Medical reports

• Psychological reports

• Occupational Therapy intake and/or progress reports

• Physiotherapy intake and progress reports

Other supporting documentation which may be required depending on the service:

• School records

• Employment records

• Income Tax records
